Summary

This week we have a brief conversation about prone mechanical ventilation for ARDS. Does it provide a benefit? When should we use it, and why does it work? I hope this is helpful as prone ventilation has been reported to provide benefit to patients with ARDS secondary to Coronavirus (COVID-19) infection.

Welcome back to ICRounds. This is Jeff Guy. This is the third conversation that I'm having with you about.

0:23.2

Treatment methodologies that may be beneficial and patients that are suffering from a COVID infection.

0:28.2

The first was a review of the surviving sepsis guidelines specific to COVID, which was an evidence-based review that looked at what evidence was available specific to COVID,

0:38.8

treatment of ARDS, or what we had learned from the SARS pandemics in years past.

0:43.9

The second is the management of ARDS, an approach to mechanical ventilation,

0:49.3

and the treatment of the hypoxemic patient.

0:52.1

This is going to be even more specific onto that, is that what is the approach to prone

0:57.1

ventilation?

0:58.8

Prone ventilation has been called out in the surviving sepsis guidelines as an approach

1:03.8

to the management of patients with ARDS and SARS.

1:07.4

For some folks, this is rather old school.

1:10.0

They use prone ventilation as their general approach every

1:13.0

day. And other patients, this will be new. I've had a lot of people come up to me and ask me,

1:17.6

well, what is prone ventilation? I understand what it is, but how does it work? It seems to some

1:22.6

folks that they would rather try other types of, quote, salvage methodologies before going to prone.

1:28.8

What I hope to show to you today is that prone mechanical ventilation does provide a survival benefit for patients with ARDS.

1:37.8

We're going to go back and look at some of the evidence that's been around for years and actually go back probably all the way back to the 70s and really

1:45.5

explain where prone ventilation got its initial start.

1:48.9

Then I want to kind of hopefully change your thinking a little bit about prone ventilation that

1:53.4

it's not necessarily an approach to be used in salvage therapy for ARDS.

1:59.2

Perhaps we need to be putting this more in the front part of our
