simple, but you can screw it up. So today, let's talk about the preamble. Josh tells you to trust

no one.

This was a really interesting podcast for me to research because I kind of thought that I really understood RAPDO and then after reading all about it, I realized that I didn't.

And I don't think anyone really, really understands this.

And it's super complicated because the problem is most of the RAPDO that we see in the ICU is

really complex.

So RAPTO usually doesn't occur on its own. It's like rhabdo plus trauma, rhabdo plus dehydration, rabble plus hypothermia.

So there's multiple things going on.

And then the question is how much of the A.K.I.

How much the kidney injury is due to the rhabdom?

My life is how much of it's due to the dehydration?

How much is due to the trauma. So it's just very hard to sift these things out.

It's not the classic rhabdo we've seen emerge where some 30-year-old overweight dude like

myself goes to a spin class for the first time and just gets a little exertional compartment

syndrome, shall we say, Josh.

This is ICU level rhabdo.

So tell me, evidence-based, not evidence-based? What is the state of the literature around treatment?

There is basically no solid evidence on treatments at all.

There are some RCTs that basically show no benefit or particular difference in various therapies.

But there's really no solid evidence on treatment.
